Youth Villages – LifeSet
LifeSet is a statewide program that takes an individualized approach. It is an evidence-informed model that pairs a highly trained specialist with each young person to help identify and accomplish goals. The LifeSet program supports young adults with employment, education, housing, money management, health care, and transportation goals as well as provides emotional support and connection to community resources. The target population is young adults ages 17-22 who are currently in foster care or have a history of being in foster care after the age of 16. The overall program seeks to monitor and support the youth with case management services to ensure they are meeting goals and also connecting them with appropriate services to best support their needs.
